The CSC Eligibility Card, which will serve as an official proof of civil service eligibility, is now initially available to passers of the 2015 Civil Service Examinations, this was announced yesterday by the Civil Service Commission (CSC). The CSC added that the said eligibility card will specifically be available for passers of the 2015 Civil Service Exam in Paper and Pencil Test (CSE-PPT) or Computerized Examination (CSC-COMEX) onwards.

Claiming the CSC Eligibility Card

CSE Passers and other CSC-administered tests after 2015 will be issued a Temporary Certification of Eligibility printed with no pay and they may also apply for a Regular Certification of Eligibility (Click here for more details).

REQUIREMENTS:
- 1 (One) Valid ID (Click to check the List of Valid ID’s accepted by the CSC)
- Temporary Certification of Eligibility printed on CSC letterhead
- Regular Certification of Eligibility
- Application Receipt or CSC Official Receipt
- Payment of PhP200.00

To get the card, 2015 CSC passers should personally claim their card at the CSC Central Office (CO) or the CSC Regional Office (RO) where the eligible took the examination.

CSC Passers currently residing in or moving to another region, or remote regions within the region. They must send a written request to the CSC RO with jurisdiction over the test site where they took the test, requesting that the card be forwarded to their preferred CSC RO/FO conduit. In their written request, they must also cite their reasons for the request then they may personally claim their Eligibility Card at the CSC Regional or Field Office (conduit CSC RO/FO) nearest their place of residence or work. Please be reminded that you must personally claim the Eligibility Card because representatives are not allowed to claim it on your behalf.

Replace Card in case Error or Loss

Error and/or Correction of Personal Information (COPI)

Mistaken encoding by the CSC Regional Office regardless of whether or not the eligible has made suitable corrections on the Examanee Attendance Sheet (EAS), provided, the correction being requested once, upon validation, consistent with the eligible’s data appearing on the Application Form. In this case, the eligible is no longer required to pay COPI and the CSC Eligibility Card fees.

Change of name exclusively for female eligibles only, resulting from change of civil status from single to married, or vice versa. In this instance, the eligible shall no longer be required to pay the COPI fee. However, the eligible shall pay the eligibility card processing fee.

Dissimilarity of data/information of eligible appearing on the Birth Certificate. In this case, the eligible shall be required to pay the fees for both the COPI and the eligibility card.

REQUIREMENTS:
- Original CSC Eligibility Card
- Application Receipt or CSC Official Receipt
- Written request to the CSC Regional Office concerned (in case of CSE-PPT and CSC-COMEX held at the Regional Office), or the CSC CO (in case of CSC COMEX held at the CO)
- Filled out COPI form
- Original documents such as birth/marriage certificate and legal papers needed.

Loss of Card

In case of loss of card, the eligible may apply for another issuance of CSC Eligibility Card.

- Written request to the CSC Regional Office concerned (in case of CSE-PPT and CSC-COMEX held at the Regional Office), or the CSC CO (in case of CSC COMEX held at the CO)
- Original Notarized Affidavit of Loss of Card
- Original and photocopy of two (2) Valid IDs (Click to check the List of Valid ID’s accepted by the CSC)
- Certification and/or Authentication of Eligibility.

NOTE:

CSC Eligibility ID for Professional and Sub-Professional are available for May 3, 2015 CSE passers only. Please wait for further announcements on the availability of CSC Eligibility ID for other passers.

Other Special Eligibility

The CSC for some time now has been granting some different eligibilities under special laws to qualified individuals. You may be qualified for the grant of any of these eligibilities. For eligibility granted under special laws and CSC issuances, the authority to release the CSC Eligibility Card shall be based on the Jurisdiction of the CSC Regional Office, as follows:

- Honor Graduate Eligibility: University or college from which an applicant has graduated

- Scientific and Technological Specialist Eligibility: DOST Head Office

- Electronic Data Processing Specialist Eligibility: CSC Regional Office/Field Office

- Sanggunian Member Eligibility: City, municipality , or province where an applicant has rendered services as member of the Sangguniang Panlungsod/Bayan/Panlalawigan

- Foreign School Honor Graduate Eligibility: Place where an applicant is currently based in the Philippines, or the CSC RO of choice for an applicant who is based abroad

- Barangay Official Eligibility: Barangay where an applicant has rendered services

- Barangay Health Worker Eligibility: Barangay where an applicant has rendered services

- Barangay Nutrition Scholar Eligibility: Barangay where an applicant has rendered services

- Skills Eligibility Category II: Agency where an applicant has completed one year of very satisfactory actual work performance under temporary status of appointment

- Veteran Preference Rating Eligibility: CSC Regional Office/Field Office

The CSC CO through the Integrated Records Management Office, or the CSC Regional Office shall administer the certification and/or authentication process  for  eligibilities resulting from passing the corresponding examinations and for eligibilities granted under special laws and CSC issuances, guided by existing policies on certification and/or Authentication of Eligibility.
